Classes added after grade rosters are uploaded to the Web are exceptions to the last day to drop policy and require:
  • A Registration Transaction Form signed by the student, the instructor, and the division dean
  • A Grade Change Form signed by the instructor and division dean
  • The Grade Change Form must include the grade the student earned in class.

WINTER QUARTER Web grade rosters available to instructors
To add a class after grade rosters upload to the Web:
  • The student must complete a Registration Transaction Form and obtain the instructor's signature. The student delivers the completed Registration Transaction Form to the Registration Office in Building 15.
  • The instructor must complete a Grade Change Form indicating the student's grade in the class. The instructor delivers the Grade Change Form to the Transcript Office in Building 15, or sends it via campus mail to MS 2151.
